Consultation Appointment Steps

To schedule a consultation appointment with Dr Kuhn, please complete the following steps:

1. Click on the Payment button below to initiate a payment. This action will cause the Medical Form to be displayed. Please wait a moment for Medical Form to appear.

2. Fill out the Medical Form and click on Submit

3. This action will notify Dr Kuhn

When Dr Kuhn has received the Medical Form and Payment for an initial consultation, Dr Kuhn will contact you via email to set up an appointment time.

Initial Consultation – $150 (60 minutes)

Follow-up Consultations – $125 per hour.
